The rule of thumb here is that if two-thirds of the original tooth structure is lost or compromised, a crown becomes necessary. This applies not only to the top of the tooth but also to the sides where the filling extends. The reason behind this recommendation is that, in most cases, if a filling is too large and the tooth is not adequately supported, it is at risk of breaking.
